Health Insurance information

If your insurance company requires a referral you will need to contact your Primary Care Physician (PCP) to get the initial referral prior to your first visit. Please bring your insurance card with you to your appointment so we can make a copy of it for your file.

Motor vehicle accident insurance information
If you were in a motor vehicle accident you have three options for payment.
- File a claim with your personal injury protection car insurance.
- Use your health insurance.
- Pay out of pocket.
We do not accept 3rd party insurance, such as car insurance from another driver. We do not accept letters of protection from attorneys. If you have questions regarding how to pay for your treatment due to a motor vehicle accident please contact us.
Worker’s Compensation Insurance information
If you were injured at work and would like to use worker’s compensation to pay for your treatment you will need to do the following:
- Inform your employer of the accident as soon as possible.
- File a claim with worker’s compensation.
- Inform work comp and receive authorization to see Dr. Seghi for treatment.
We will need the following information from you when you call to make your appointment:
- Name and phone number of the Work Comp Insurance Company.
- Name and phone number of your employer.
- Date of Accident.
- Authorization number from Work Comp if available.
Please note that we will have to verify with worker’s compensation that they have authorized you to receive treatment from Dr. Seghi. If you have questions regarding your treatment due to a work related injury please contact us.
